Dear Adoptive Family,

Congratulations!  We are delighted that you have selected Axis to work with you on your adoptive home study.  The following items are necessary to complete your home study (each can be found on our website as links you could download at www.axisadoption.org):  1) Home Study Application, 2) forms and releases for the background checks from the Florida Department of Children & Families Child Abuse Registry (Central Abuse Hotline Record Search),
3) Child Abuse Hotline Searches from all states in which you have resided since the age of 18, 4) FBI Fingerprint clearances, 5) Florida Department of Law Enforcement background clearances, 6) the Adoptive Parent Questionnaire, 7) physical forms, 8) financial statement, 9) employer references and 10) personal references. 

Please return the Home Study Application, agreed upon home study fee, the background check forms, signed releases of information and a check made payable to "Axis" for the agreed upon fees.  We will begin to process your background clearances and you will then be contacted by an Adoption Specialist to schedule your home visit(s). 

The background clearances take the longest to process.  You will want to get your fingerprint cards submitted to the FBI immediately.  Follow the directions on the letter regarding FBI fingerprint clearances.  Another priority is the DCF Child Abuse Hotline Record Search.  Please submit these forms to Axis as soon as possible.

The questionnaire will require more of your time and thought.  If there are any questions that you have concerns about and you would prefer to discuss them during our interview, please make a notation beside or highlight the question(s).  Please return it to us with the remaining documents.  Prior to the completion of the home study we will need the following to be completed and provided:  (1) a current physical completed within the last twelve (12) months, (2) a copy of your marriage license, (3) a copy of any divorce decrees, (4) a copy of your birth certificate, (5) a copy of your driver’s license, (6) financial statement, (7) a copy of your most recently filed income tax return (first 2 pages only), (8) the completed reference letters, (9) originals (on official letterhead) of local law background checks which you will obtain from your county sheriff’s department and local police department, (10) certified copies of any arrest records including the original police report, dispositions and follow up information, and a written explanation of the incident in your own words, and, (11) a copy of your military discharge documents (if applicable).
