Dear Adoptive Family,

Congratulations!  We are delighted that you have selected Axis to work with you on your adoptive home study.  We are enclosing the following items: (1) Home Study Application (2) the agreed upon Engagement Contract (3) Fee Schedule and Services (4) forms and releases for the background checks from the Florida Department of Children & Families Child Abuse Registry (Central Abuse Hotline Record Search) and the Florida Department of Law Enforcement (5) the Adoptive Home Study Questionnaire, (6) physical forms, (7) financial statement, and (8) employer and personal reference forms.  Please return the Home Study Application, agreed upon home study fee, the background check forms, signed releases of information and a check made payable to “FDLE” for $24.00 for each adult to be cleared.  We will begin to process your background clearances and you will then be contacted by an Adoption Specialist to schedule your home visit(s). 

The questionnaire will require more of your time and thought.  If there are any questions that you have concerns about and you would prefer to discuss them during our interview, please make a notation beside or highlight the question(s).  Please return it to us with the remaining documents.  Prior to the completion of the home study we will need the following to be completed and provided:  (1) a current physical completed within the last twelve (12) months, (2) a copy of any marriage license, (3) a copy of any divorce decrees, (4) a copy of your birth certificate, (5) a copy of your drivers license, (6) the financial statement, (7) a copy of your most recently filed income tax return (first 2 pages only), (8) the completed reference letters, (9) originals (on official letterhead) of local law background checks which you will obtain from your county sheriff’s department and local police department, (10) certified copies of any arrest records including the original police report, dispositions and follow up information, and a written explanation of the incident in your own words, and, (11) a copy of your military discharge documents (if applicable).  For all Domestic home studies, we will need a copy of your Adoption Disclosure Statement (63.085 F.S.) that was provided to you by your attorney.  For International home studies, please include copies of your passports.  Please also include a recent photograph of your family.
